This appointment is for an adult applying for a Australian passport using the 5 page overseas passport application form:
- You are applying for a first ever Australian passport
- You are changing your name or personal details on the passport
- You are applying for a first adult passport and your child passport was issued to you at 15 years or younger
- You are replacing a lost or stolen passport
- You are applying to replace a limited validity passport, emergency passport, or other temporary travel document
- Your last passport was issued before 1 January 2006

Incomplete or incorrectly completed applications cannot be accepted. Application requirements are determined by Australian legislation and Australian Passport Office policy, and these requirements cannot be waived by Passport Office staff.
